  • Reduce Social Isolation Among Patients
  • Determine Eligibility And Enroll Individuals In Health Insurance Plans

Requirements:

  • Has Obtained A Health-Related Bachelor’S Degree
  • Help Patients Navigate Healthcare And Social Service Systems
  • Strong Interpersonal Communication And Relationship-Building Skills
  • Understanding Of Local Social And Health Resources To Make Appropriate Referrals
  • Ability To Maintain Electronic Medical Records And Manage Data
