- Reduce Social Isolation Among Patients
- Determine Eligibility And Enroll Individuals In Health Insurance Plans
Requirements:
- Has Obtained A Health-Related Bachelor’S Degree
- Help Patients Navigate Healthcare And Social Service Systems
- Strong Interpersonal Communication And Relationship-Building Skills
- Understanding Of Local Social And Health Resources To Make Appropriate Referrals
- Ability To Maintain Electronic Medical Records And Manage Data
